Sample size table

I am reviewing a Design Verification Plan and there is a table attached that is used to pick sample sizes for the various tests being performed. The table apparently has been in other documents and has continued to be used. I am trying to figure out where it came from, so we can put a reference source in the document. It is titled "Sample sizes binomial probability assumed". The information in the table is: confidence coefficient % (50,60,70,80,90,95,99), percent reliability (75,80,etc.), and number of failures allowed in sample (range from 0 to 3). Can anyone tell me where this comes from? Thank you Eric Larson
