The standard calls for testing all 1250.

(2) Is this 21 units per 100 , or is it 21 from the whole 1250?

It is 21 units per 1250

If your quality characteristic is a measured variable, you could consider using a variables plan with its OC curve matched to the attribute plan. The plan would offer equal protection/probabilities. For the n=1250, Ac=21 plan, the matched variables plans are:

n=437, k=2.12 (SD Unknown)

n=135, k=2.12 (SD known)

The newest version of TP105 (v3) will calculate the variables plans that match the producers and consumers points of any attribute plan.
